EAP GRANT APPLICATION

Hawthorn Community Chest is proud to partner with Inner East Community Support (IECS) through the Education Assistance Partnership (EAP). IECS generously provides the funding for EAP Grants, helping us support secondary students living in Boroondara. We are grateful for their ongoing commitment to education access and equity.

 

To apply for an EAP Grant for the 2026 academic year, please complete the following form by 7 November 2025. This form should be filled out by the Student Advocate (such as a care provider, case worker, or school wellbeing coordinator) together with the student, and with permission from the student’s parent or carer.

  • I understand that HCC collects personal information (such as name, date of birth, and address) to assess eligibility for financial support grants. I acknowledge that without this information, the application may not be assessed.

  • I confirm that the student’s parent/carer has provided consent for me to apply for an Education Assistance Partnership (EAP) Grant on the student’s behalf, and for HCC to collect, use, and disclose the student’s personal information as outlined below.
